Services Provided :

We perform consultations and ex6aminations for the diagnosis and treatment of arthritis and related conditions. We also offer bone densitometry, drug infusion, FAA Aviation medical examination and neuromextrix nerve conduction study. Patient education and clinical research are an important part of our practice. X-Ray and Lab Services are also available, at or near our facility.

Office Financial Policy :

We are aware of the need for a definite financial understanding between doctor and patient regarding payment for medical care. The payment of fees is the direct obligation of the patient. All patients are required to resolve any payments owed on their account at the completion of their office visit. Special needs are understood and it may be necessary to set up a payment plan for patients requiring extensive treatment. Some insurance plans require only a co-payment. Information about your insurance obligation may be located on the reverse side of your insurance card. Please inform the receptionist if you require assistance.

By Appointment Only :

As a necessity, doctors must work by appointment. Every effort will be made to see each patient in the earliest possible time frame. Should a situation arise where immediate treatment is needed, our staff will make a strong effort to schedule a same day appointment. If you are unable to keep an appointment, we ask that you contact our office within 24 hours in order for our staff to utilize your time period for another patient.

New Patient:

WELCOME! - If you are visiting our office for the first time, we will need to obtain some vital information. Our receptionist will ask your to complete an information form and a medical history form. Forms are necessary for the doctor to obtain essential data about you.

All information given to our office is confidential and protected by the Privacy Act.

Emergencies:

After hours, weekends, and on holidays, there will be an answering service available in case of an emergency. Please limit after-hours calls to emergencies only. Thank You.
